Community Hospice and Homecare RN, Full-Time

Our Lady of Peace Community Hospice and Homecare is seeking a full-time RN to provide care to patients in a variety of community settings within the Twin Cities Metro area. Candidate will participate in team meetings to establish patient plans of care. Candidate will see both hospice and homecare patients. Monday-Friday 8am to 4pm + every 7th week “on call”. Laptop or iPad plus iPhone provided for documentation needs. Mileage reimbursement includes travel time.

Community Hospice Home Health Aide

Our Lady of Peace Community Hospice and Homecare is seeking a Home Health Aide for 32-40 hours per week patient End of Life Care. Candidates must have a current Minnesota HHA or CNA license. Candidates must have reliable transportation and be CPR Certified. Previous home care experience is required. This position is benefit eligible and we reimburse for mileage. Educational grants are available. Candidates must possess a high level of team capabilities and excellent communication skills.

– FULL TIME + BENEFITS = M-F + Every Other Weekend, 6am-2:30pm
– PART TIME M-F 2pm to 6:30pm (2 shifts or so per week, days will vary) + two 8 hour shifts every other weekend

The Dietary Aide alongside of the Cook will be responsible for the preparation and execution of resident meals (21 residents). The Dietary Aide performs designated work and cleaning routines for the Dietary Department. The Dietary Aide will ensure that meals are prepared and presented with excellence as to provide quality customer service.

Duties to include:

– Setting up and clearing trays

– Assist in loading food trays and serving meals

– Meet scheduled meal times

– Wash and store dishes

– Sweep kitchen floors and take trash out


– Knowledge of sanitation, food safety, and kitchen equipment

– Must be punctual, have a positive attitude, and work well with a team

– 6+ months experience preferred.
