You have a choice in hospice care providers. Our Medicare-Certified Hospice Residence provides free, comprehensive hospice care at our location in Saint Paul.

Comprehensive Care for All

Our Lady of Peace provides care to adults with any terminal illness. We are also open to patients of all faiths and walks of life; all are welcome here.

Our dedicated care teams work closely with patients and their families to develop an individualized care plan that suits their needs. Because coordination of services is so important, each care team consists of a physician, nurse, social worker, pastoral care director, bereavement services, and pharmacist. Bringing these resources together under one roof increases communication, reduces response time, and keeps patients and families involved in care decisions.

Spiritual Support

Our Lady of Peace provides a spiritual assessment to design a plan to meet each patient’s unique spiritual needs. Patients, family, and friends can also attend Mass and take Communion in our beautiful chapel.

Adult Admission Criteria to Our Home

The following criteria must be met prior to adult admission to our home at Our Lady of Peace.

—Evidence of terminal disease state as shown by past medical history, lab work, scan reports (CT, MRI or PET), and functional or clinical status.

—Karnofsky Performance Status of less than 30–40%, or Palliative Performance scale of less than 40%, or Functional Assessment Staging of 7D or less. General prognosis of one month or less.

—Caregiver and patient support for cessation of all curative treatment with a DNR, POLST, or advance directive that does NOT include partial resuscitation attempt, IV interventions, chemo of any type, radiation, transfusions, dialysis, or assisted breathing.

—Applications for admission will be submitted for prior approval. We reserve the right to screen admissions for persons that wish to use medications, and or treatments that are not part of our formulary or medical approach, or are otherwise deemed inappropriate for this environment.

—Factors that would prevent admission here include; ambulatory dementia patients, persons with a history of recent violence, incarcerated persons, persons with behavioral issues not related to the terminal disease state, bariatric patients above 250 lbs. (or expected to be due to disease state), persons with an unstable or complicated mental health history, and those persons with an infectious condition that may put other persons at risk in our environment (isolation rooms are not available here).

Children's Admission Criteria to Our Home

Children who have been diagnosed with terminal cancer must have written referrals from our care partners. Please contact us for more information.

—Do Not Resuscitate (DNR) / Do Not Intubate (DNI) / Allow Natural Death, application signed by Physician or Nurse Practitioner
—Most recent medication list
—Recent history and physical or progress note with past medical history, co-morbidities, and evidence of terminal disease state such as recent CT/MRI/PET scans
—Copy of Advance Directive, if present
